Child Health Associates All Forms Must Be Completed Through MyChart

Starting May 1, 2025, all required forms for well visits (patients under 13) must be completed in MyChart before your child’s appointment.

This new process will help reduce wait times, ensure accurate information, and make your visit smoother and more efficient.

To complete the forms:

1. Log in to your MyChart account.
2. Navigate to your child’s upcoming appointment.
3. Complete all pre-visit forms at least 24 hours before the visit.
